Sorry you’ve been battling this problem with your cat. From what you describe it certainly sounds allergy related. It’s important to remember cats can be allergic to anything and many times it is worse in the winter if the cat has allergies to things indoors like dust or dust mites. I know you’ve done a lot already but it may be worthwhile to consider a biopsy, especially if his nail beds are affected (many auto-immune diseases can affect the nail beds). If not done I would also have him checked for yeast and ringworm. I know he’s been on special diets as well but I always like to revisit this to be sure there isn’t anything that was missed. Allergy testing for food allergies is helpful but the best test is a food trial. This usually means putting a cat on a “hypoallergenic diet” (usually a veterinary only diet) for nine weeks with absolutely no other food. Not a treat, not a hotdog, nothing! In the mean time as well, if he is really miserable you may want to talk to your veterinarian about treating him with an anti-inflammatory like a steroid to see if you can give him some relief. You could also consider seeing a veterinary dermatologist to help you as well. Good luck with this!
